At the EMC, we're intentional about building community. You will be meeting a lot of new people. You will learn their stories and learn a lot about yourself too.
One of the first ways we build community starts before you even arrive. We learn about each of you through our participant forms. All EMC participants should complete the three participants forms below prior to arriving at the EMC. You can complete and submit them online.

General Participant Form: Helps us learn some basic information about you.
Community Covenant: Introduces you to how we build community at the EMC.

Medical Form: Lets us have important information to keep you healthy and support you if there's a medical emergency.
Sessions V and VI:
One of our partner sites is an organization called MANNA. Some missioners from Sessions V and VI will be volunteering at this site and they require volunteers under 18 to have a parental consent form. Parents of participants between the ages of 14 and 18 must complete this parental consent form:
